At Penn Medicine, improving health in a meaningful and lasting way is at the heart of what we do. Our commitment to serving and caring for communities across 27 counties in Pennsylvania and New Jersey drives the passion and creativity of our dedicated physicians, nurses, faculty, trainees, students, staff, and volunteers. Through our regional locations—Penn Medicine Chester County Hospital, Penn Medicine Lancaster General Health, Penn Medicine Princeton Health—our Philadelphia-based hospitals, and our numerous clinics and outreach programs, we work to ensure high-quality, accessible care for all.
We recognize that health is influenced by more than just medical treatment. It is deeply connected to the environment in which people live, work, and play. That’s why we engage in initiatives that address food insecurity, housing safety, maternal and child health, mental well-being, and chronic disease prevention. Our teams vaccinate children against infectious diseases, offer screenings for conditions such as hypertension and cancer, and provide critical support for those struggling with substance use disorders.
Through partnerships with local organizations, we meet people wherever they are in the community to break down barriers to care and create sustainable health solutions. Our mission to serve our neighbors is strongest when we work together, combining our expertise, resources, and community partnerships to improve lives.

Understanding our communities’ needs
We regularly evaluate the changing health needs of our communities through the Community Health Needs Assessment (CHNA). Every three years, we conduct this in-depth assessment to gather data about the unmet health needs in the local communities we serve. This information enables us to customize our programs and services to address key health concerns, improve accessibility, and enhance the overall well-being of the community.
From this needs assessment, a corresponding Community Health Implementation Plan (CHIP) was developed for the specific needs of the community our Philadelphia-based hospitals serve.
