To make a referral to any Penn Medicine at Home entity, please call us toll-free at 866-888-8598. Referrals can be made directly through PennChart and PhysicianLink for those with EMR access.
Physicians, please have the following information ready when referring a patient for Penn Medicine Home Health:
- Required disciplines (skilled nursing, physical therapy, occupational therapy, etc.)
- Significant past medical history
- Performed procedures
- Faxed or verbal orders (please note that orders should be faxed from a physician’s office if there is a prescription)
- If wound care is required: measurement of wound
- Specific instructions for specific treatment modalities
In order to help expedite your call when referring patients to Penn Medicine Home Health, please have the following information when you call Central Intake:
- Demographic information (such as patient name, address, and telephone number)
- Patient’s emergency contact information
- Diagnosis
- Name of the ordering physician
- Name of the physician who will sign orders (if different from ordering physician)
- Patient height, weight, and allergy information (where applicable)
- Inpatient stay dates
- Insurance information (Including secondary, if applicable)
- Desired start of care date
