Strength through service

From operating neighborhood clinics to mentoring youth to helping patients manage medical appointments and cook meals, Penn Medicine team members go above and beyond to address health disparities and show up for our communities.

Community Health Needs Assessment and Implementation Plans

In compliance with the Patient Protection and Affordable Care Act, Penn Medicine's hospitals (Hospital of the University of Pennsylvania, Penn Presbyterian Medical Center, Chester County Hospital and Pennsylvania Hospital) completed their most recent Community Health Needs Assessment (CHNA) in 2022 in partnership with public health departments, other area health systems and a robust network of community organizations and residents.

From this needs assessment, a corresponding Community Health Implementation Plan (CHIP) was developed for the specific needs of the community our Philadelphia-based hospitals serve. Through these targeted efforts, Penn Medicine continually moves forward toward health equity.
