Penn Medicine understands how important it is for patients to have access to their health information and to be able to share it with others.
We offer multiple options to access and share your health information, including via the patient portal or written authorization. Learn more about each option below.
Patient portal
In the portal, you can readily and securely access much of your health information at your convenience and for free. You can also download a PDF copy of your health information for yourself or for you to provide to other health care providers.
- Penn Medicine, Chester County Hospital and Penn Medicine Princeton Medical Center patients, log in or sign up for an account.
- Lancaster General Health patients log in or sign up for an account. If you have issues or need assistance with MyLGHealth, please call 717-544-4357.
The patient portal also offers a Share Everywhere feature that lets you give providers one-time, limited access to your health information that is in the portal, such as your medications, allergies, immunizations and other health issues.
Health Information Management
You can request a copy of your health information via the patient portal as stated above.
If you want records to be sent to another health care provider or someone other than yourself, you must submit a written request and may do so by using the form below.
Authorization form
If you would like to use the Penn Medicine form to submit your written request for records, please use the following link to access the authorization form complete it, and mail or fax it to the hospital or practice where you would like to request records.
In line with set fees under federal and state law, we may charge you for a copy of your medical records.
