Supporting Older Adults (SOAR) at Risk: Primary Care Geriatric Nurse Practitioner (GNP) Co-Management

Primary Care Geriatric NP Co-Management is a partnership between primary care geriatric nurse practitioners (GNPs) and an acute care inpatient team, with the goal of identifying and addressing common geriatric syndromes amidst the acute care episode, as well as supporting continuity of care across the continuum.

The SOAR Program provides comprehensive care for high-risk older adults. This innovative program brings a primary care approach to ensure timely and efficient implementation of evidence-based geriatric care during hospitalization and promotes continuity across settings of care. The GNPs collaborate with interdisciplinary teams to address common geriatric syndromes during hospitalization and anticipate post-acute needs. The GNPs understands the comprehensive longitudinal complexity of older adults and places the exacerbation of chronic disease and hospitalization episode in context. The NP considers and can directly address the post-acute ongoing needs to ensure maximal outcomes. GNPs conduct a comprehensive geriatric assessment (CGA) to identify areas of risk and opportunities for intervention. The NP discusses findings and recommendations with the interprofessional team, and will write or change orders in accordance with the team’s agreed-upon plan of care. GNPs also oversee Student Geriatrics Associates in the delivery of interventions aimed at preventing delirium and functional decline. We take a person-centered approach to provide consistent high-quality care to older adults and their loved ones.

ASPIRE program

The ASPIRE Program at Hospital of the University of Pennsylvania (HUP) is an enrichment program designed for high school juniors interested in pursuing a career in nursing. Upon acceptance, ASPIRE Scholars participate in a number of interactive experiences designed to expose and introduce them to the profession of nursing. After successful completion of the high school program, ASPIRE Scholars receive a stipend of $500, and will be invited to return to participate in the ASPIRE Program their senior year.

Following their senior year, ASPIRE Scholars will have the opportunity to receive scholarship funding to attend La Salle University to obtain a Bachelor’s of Science in Nursing degree and also maintain part-time employment at HUP. ASPIRE Scholars will receive ongoing support and mentorship throughout their undergraduate program and employment at HUP.

The ASPIRE Program is committed to making the nursing profession accessible for those who might never have considered a nursing career possible. The ASPIRE Program is supported by the Howley Foundation and La Salle University.
