Senior HealthLink (SHL), a free and voluntary program within Penn Medicine at Home, offers preventive healthcare to senior citizens that live independently. The program promotes the maintenance of optimal health, independence, and self-management of chronic medical conditions through several services.
Why is this important?
Today's older adult population is living longer and faced with challenges related to multiple and chronic health conditions that impact personal independence, self-determination, and quality of life. SHL may identify and assist at-risk individuals between doctor appointments, after discharge from a hospital, or skilled home care visits to avoid unnecessary hospitalizations or emergency room visits.
Program Services
Home Visits
Upper level nursing students, supervised by nursing instructors, the SHL program director and SHL staff, visit older adults residing in or near the Chester County, PA region for:
- Basic health assessments
- Blood pressure and pulse checks
- Medication reviews
- Health surveys
- Health Education
- Home Safety Assessment
- Community resource information
- Social Visit
Telephonic Visits
SHL offers preventive health education and assessments over the phone, as well. This service is available to any current SHL client or older adult living in the geographical area served by Penn Medicine.
