Penn Home Care and Hospice Services (HCHS) has been an essential component of the continuum of care for Penn patients for many years, acting as an extension into patients’ communities and homes. With its growth and added agencies throughout the region, the organization has decided to consolidate home health and hospice services. By integrating its home health and hospice partners, HCHS will be able to offer more standardized and coordinated care, while enhancing patient experience.
The first step of this integration will occur on March 10th, when the organization will integrate Neighborhood Home Health and Hospice with Penn Care at Home and Wissahickon Hospice. Corresponding with this integration will be an overall rebrand of services to ensure that patients, families, and care teams know, without a doubt, they are receiving care from Penn Medicine.
As of March 10th, the following changes will occur:
Along with Penn Center for Community Health Works and Penn Home Infusion Therapy, all of these services will become part of the newly re-branded entity Penn Medicine at Home!
Penn Medicine’s agencies in Princeton as well as Affilia in Lancaster will also rebrand to these same naming conventions in the coming months.
