In addition to the above therapies, Penn Medicine Home Health also provides social workers to patients requiring additional care and resources. Our social workers assess and support the individualized needs of each patient and family, ensuring they have access to the information, care, and resources they need.

Follow Up Care a Top Priority

Although rehabilitation at home is more comfortable and less stressful than rehabilitation in a hospital or outpatient facility, we understand patients needing care may still have questions and concerns. Our staff will contact a referred patient as soon as possible to schedule an appointment and make sure all their questions are answered.

Penn Medicine Home Health care teams provide patients and their caregivers with the tools they need to perform rehabilitative procedures correctly, make appropriate lifestyle changes and provide proper care for their loved one. Penn Medicine Home Health service availability and limitations

Service availability

Penn Medicine Home Health provides the following services: skilled nursing, physical therapy, occupational therapy, speech-language pathology, home health aide, and social worker. To qualify for home health services, a patient must be under the care of a physician or allowed practitioner who is responsible for the home health plan of care.

Service limitations

Penn Medicine Home Health (PMHH) does not provide services for patients under 18 years of age, maternal/obstetrical/post-natal services, psychiatric/behavioral health services, and does not provide care for patients who we are aware of are actively using or are under the influence of drugs or alcohol. Penn Medicine Home Health does not provide care where equipment required exceeds the competence of our staff including but not limited to ventilators. We cannot provide care if our staff is required to manipulate or manage specialized equipment, and staff is not trained in the use of the specialized equipment. PMHH does not provide care for patients who have been in violation of the UPHS Code of Conduct, including those that have a history of violence, abusive or threatening behavior towards staff. PMHH does not provide service for patients who require more than once per day nursing visit and/or when there is no caregiver available to provide care. Our services are provided in the following counties: Philadelphia County, Chester County, Montgomery County (limited zip codes), Berks County (limited zip codes), Bucks County, Delaware County, and Lancaster County (limited zip codes).
