Located at Penn Medicine Rittenhouse on South Street, the Penn Center for Connected Care is one of the largest telehealth centers in the country, providing access to critical care physicians and nurses, family medicine nurse practitioners and care coordinators. Delivering connected health care services across the region in PA, NY, and Delaware, these services operate across the care continuum, including in primary and consultative management, acute and chronic care, and in the hospital and outpatient setting.
