How to apply for a Family Medicine and Community Health fellowship program.
Our residency program participates in the National Resident Matching Program. We sponsor only J1 visas. Applications are accepted only through ERAS. The application deadline is annually on October 1.
A submission packet must include:
- ERAS Common Application Form
- Medical School Transcript
- USMLE Transcript
- Valid ECFMG certificate if graduate of non-LCME school
- Three (3) letters of recommendation
- Personal statement explaining your interest in our program
