PGY2 Ambulatory Care Pharmacy Residency Program Director

Learning Experience(s): Internal Medicine

Diane Kim, PharmD, BCACP is a Clinical Pharmacist in Ambulatory Care at Penn Presbyterian Medical Center (PPMC). She earned her Doctor of Pharmacy Degree at Rutgers University. She completed her PGY1 Community Pharmacy Residency at Duquesne University in Pittsburgh, PA and her PGY2 Ambulatory Care Pharmacy Residency at Christiana Care in Newark/Wilmington, DE.

Dr. Kim started working at PPMC in 2019 and currently practices in an Internal Medicine clinic. Her clinical interests include diabetes, hypertension, and substance use disorder. She is an active member of ASHP where she serves as a member of the Section of Ambulatory Care Practitioners Advisory Group.

Natalie Blaine headshotNatalie Goode, PharmD, BCACP

Learning Experience(s): Administration

Natalie Goode, PharmD, BCACP is the Clinical Coordinator of Ambulatory Clinical Pharmacy Services at Penn Presbyterian Medical Center. She earned her Bachelor of Science in Biology and Doctor of Pharmacy degrees from the University of Iowa. She completed her post-graduate residency training at Penn Presbyterian Medical Center in 2017.

Dr. Goode began her career as a Clinical Pharmacist in Ambulatory Care covering Oncology, Cardiology, Neurology, and Family Medicine over five years of clinical practice. She then moved to her current role as Clinical Coordinator and provides oversight of PPMC’s growing pharmacy footprint in ambulatory clinical practice. Natalie currently serves on the American Society of Health-System Pharmacists (ASHP) Council on Pharmacy Practice (CPhP) and the National Association of Specialty Pharmacy (NASP) Education Committee.

Jasmine Delk headshotJasmine A. Delk, PharmD

Learning Experience(s): Specialty Family Medicine

Jasmine Delk, PharmD graduated from the Philadelphia College of Pharmacy in 2020. She completed a PGY-1 Pharmacy Residency at Xavier University of Louisiana College of Pharmacy in New Orleans, LA in 2021. Prior to this position, she completed a PGY-2 Ambulatory Care Pharmacy Residency at UPMC Presbyterian Hospital in Pittsburgh, PA with a concentration in underserved care and global health. Dr. Delk currently works as a Clinical Pharmacist in Ambulatory Care in Family Medicine. Her professional interests include providing care to underserved populations, identifying social determinants of health, and the role of pharmacists in resource-limited settings.

Jenny Boluwaife headshotBoluwatife (Jenny) Familusi, PharmD

Learning Experience(s): Hematology/Oncology

Jenny Familusi, PharmD graduated from Jefferson College of Pharmacy in 2020. She completed her PGY-1 Pharmacy Residency at Plainview Hospital – Northwell Health in Plainview, NY in 2021 and her PGY-2 Pharmacotherapy Residency at Capital Health Regional Medical Center in Trenton, NJ.

Dr. Familusi joined Penn Presbyterian Medical Center in August 2022 as a Clinical Pharmacist in Ambulatory Care with a focus in Hematology/Oncology. Her professional interests include academia, medication adherence and patient advocacy.

Chris Hvisdas headshotChris Hvisdas, PharmD

Christopher Hvisdas, PharmD is a Clinical Pharmacist in Ambulatory Care at Penn Presbyterian Medical Center. He earned his Doctor of Pharmacy degree from Jefferson College of Pharmacy and completed his post-graduate residency training at Thomas Jefferson University Hospital.

Dr. Hvisdas started working at Penn Presbyterian in 2017. His current areas of practice include Endocrinology, Pulmonology, and Allergy/Immunology. In 2020, he was made lead pharmacist of the Penn Bone Center. His interests include Osteoporosis, Diabetes, Pulmonary Arterial Hypertension, Asthma, and COPD. He is an active member of ASHP serving in the past as a section advisory group member. He remains academically involved with research, publications, and as a lecturer for pharmacotherapy for advanced practitioners.  

Michael Silvey headshotMike Silvey, PharmD

Learning Experience(s): Rheumatology

Michael Silvey, PharmD is a Clinical Pharmacist in Ambulatory Care at Penn Presbyterian Medical Center. Michael graduated from the University of Minnesota College of Pharmacy in 2017. He has an interest in psychiatric pharmacy and completed a psychiatry focus during his PharmD education at the University of Minnesota College of Pharmacy. After graduation, he worked in a wide variety of areas of pharmacy before pursuing a PGY-1 Pharmacy Residency at St. Vincent Hospital.

Mike's experience includes time in specialty retail pharmacy, traditional retail pharmacy, and at a long-term care and inpatient hybrid model at University of Connecticut Health System (UCONN Health) that serviced the Connecticut state prison system. He started working at Penn Presbyterian in 2020 and currently practices in Rheumatology.

Swana Thomas, PharmD, MPH, is a Clinical Pharmacist in Ambulatory Care at Penn Presbyterian Medical Center. She earned her Doctor of Pharmacy degree from the Philadelphia College of Pharmacy in 2018 and completed a PGY-1 Ambulatory Care Pharmacy Residency at Allegheny General Hospital in Pittsburgh, PA in 2019.

Her previous experience includes three years of service as a rheumatology pharmacist at Geisinger Wyoming Valley Hospital. Swana started working at Penn Presbyterian in July 2022 and her current areas of practice include Gastroenterology and Dermatology. Her interests include the advancement of pharmacy services through academia and public health initiatives.
